Pediatric vision coverage (newborn through age 18) is included on all medical plans. We also offer adult Health Net Vision PPO plans (ages 19 and older) which offer you and your employees the convenience of a large national network, plus:

  • Online provider options such as LensCrafters.com, Target Optical.com, Ray-Ban.com, Glasses.com and ContactsDirect.com.
  • Choice of any frames on hand at provider locations. This includes current styles from Luxottica who makes some of today's most well-liked frames such as:
    • Vogue
    • Anne Klein
    • Giorgio Armani
    • Persol
    • Brooks Brothers
  • Low copayments.
  • The freedom to see any provider you choose – either in-network or out-of-network and be covered under the plan.
  • Discounts of 5-15% on LASIK and PRK from U.S. Laser Network
  • No claim forms for in-network services.

What's great about Health Net's behavioral health plans is that they can be paired with a Health Net medical plan. If you prefer, they can be offered as separate coverage. And, when you link a Health Net behavioral health plan with an MHN employee assistance program (EAP), it allows you and your employees to:

Services and benefits vary (falling within federal mental health parity guidelines when they apply). Our behavioral health care programs may include:

Have you ever had pain in your lower back? Maybe some throbbing and aching in a shoulder or elbow? A visit to a chiropractor may be just what you need.

Chiropractic services address the finding and treatment of pain that involves muscles, ligaments and joints. They're practiced by a chiropractor. Health Net has teamed up with American Specialty Health Plans of California, Inc.1 (ASH Plans) to offer flexible chiropractic coverage. We make it easy because you don’t need to be referred to see a chiropractor. You're free to get care from any chiropractor in our chiropractor directory. Plus, you can add chiropractic services to any current plan.

What’s covered?

Medically needed services that include:

  • Office visits copays and maximums (vary by plan).
  • X-rays and lab tests.2
  • Annual chiropractic appliance allowance applies toward buying chiropractic items, such as:
    • Thoracic and lumbar supports
    • Cervical collars and pillows
    • Heel lifts
    • Ice packs
    • Orthotics
    • Rib belts
    • Home traction units

Acupuncture is a great way to relieve mild to chronic pain. This traditional Chinese medicine has been practiced for hundreds of years. Members can add it to any current Health Net plan.

Are you looking for relief from mild to chronic pain? Acupuncture is a form of traditional Chinese medicine in which thin needles are inserted into the body to relieve pain. It is practiced by an acupuncturist and can be very useful in helping your body to become pain free.

Health Net has teamed with American Specialty Health Group, Inc.1 (ASH Group) to offer you flexible acupuncture coverage. With this program, you're free to get care from any acupuncturist in our acupuncture directory. Plus, you can add this coverage to any current plan. Best of all you do not need to be referred to see an acupuncturist.

Offer your employees a life insurance benefits package that includes both Group Term Life and Accidental Death & Dismemberment (AD&D).

If you want, you can offer employees a life insurance benefits package! And it includes both Group Term Life and Accidental Death & Dismemberment (AD&D). Health Net has built these two products that can be added to any Health Net medical plan. With these plans, you can help your employees prepare for any life event.

The package offers the ease of working with engaged Health Net representatives. They'll help guide you through each process, which includes:

  • Sales
  • Administration
  • Claims

Health Net Life Insurance Company offers a full range of benefit schedules. They're a good fit for both small (2-50) and large (51+) employer groups.

Multiple of salary. This benefit can be based on the amount of your employee's pay each year. This type of benefit can provide a basic life amount of one year's pay – with a maximum of $50,000.

Job/job class. The group life benefit can be based on job or job title. For instance, a group of corporate leaders could be insured for a basic life amount of $50,000. All other employees may be insured for a basic life amount of $25,000.

Dependent life. Benefits can be paid to a spouse and dependent children. It can only be purchased as part of a basic group life policy. It would include these High or Low Dependency options:

Group life plan features

Waiver of premium. A life benefit may be continued for certain reasons. Examples include a short-term layoff or leave of absence. Coverage may also extend in keeping with the Family Medical Leave Act of 1993. This act allows a qualified enrollee, who has become totally disabled, to continue coverage. He or she does not have to pay a premium while being disabled.

Accelerated death benefit. This benefit provides financial safety to the insured in time of need. It also protects the beneficiary. It can be paid to an insured when a doctor confirms a terminal illness. The accelerated benefit is a part of the basic life insurance amount and is paid in a lump sum. The rest is paid to the insured's beneficiary once the insured passes away.

Conversion privilege. This commits the plan issuer to renew for a set amount of time. It doesn't matter if there are changes in the insured's health. It can be given to certain members whose coverage ends due to reasons detailed in the group's plan. It does not require a physical exam, nor does it need the insured to prove he or she qualifies. Coverage must be applied for. The first premium must be paid within 31 days from the date the coverage begins.
